Tesamorelin is a synthetic GHRH analog distinguished by an N-terminal trans-3-hexenoic acid modification that extends its half-life and metabolic stability. It is FDA-approved (as Egrifta) for the reduction of excess abdominal fat in HIV-infected adults with lipodystrophy — a condition causing abnormal fat redistribution from antiretroviral therapy. Beyond this approved indication, tesamorelin has been investigated for visceral adiposity in non-HIV populations and, notably, for cognitive decline in older adults with mild cognitive impairment. It represents the gold standard among GHRH analogs due to its clinical validation.
