Semaglutide is a synthetic analog of GLP-1 (glucagon-like peptide-1), a naturally occurring incretin hormone secreted by intestinal L-cells in response to food intake. Its unique C18 fatty diacid modification enables albumin binding, extending its half-life to approximately one week — enabling convenient weekly subcutaneous dosing. FDA-approved as Ozempic (2017) for type 2 diabetes and cardiovascular risk reduction, Wegovy (2021) for chronic weight management at higher doses, and Rybelsus for oral type 2 diabetes treatment. The SELECT trial (2023) demonstrated a 20% reduction in major cardiovascular events in non-diabetic obese adults, expanding its clinical relevance.
