IGF-1 LR3 is a synthetic analog of insulin-like growth factor 1 (IGF-1) that incorporates a 13-amino-acid N-terminal extension and an Arg substitution at position 3. These modifications reduce binding to IGF-binding proteins (IGFBPs) — which normally sequester IGF-1 and limit its activity — resulting in a dramatically extended half-life (~20–30 hours vs. ~15 minutes for native IGF-1) and approximately 60-fold greater potency. IGF-1 is the primary mediator of GH's anabolic effects, stimulating protein synthesis, satellite cell activation, and muscle hypertrophy. LR3 modification allows systemic activity that native IGF-1, which is largely bound and locally restricted, cannot achieve.
